Case 4 History ---- At age 61 years, this patient’s voice became softer and slurred; he began shuffling, stumbling and falling backward which dramatically worsened. He developed emotional lability, laughing inappropriately with little provocation. By age 63, swallowing was worse, handwriting became smaller and less legible, and more angled. His face was masked and less animated. By age 65, he could still perform his ADL. In the last 6 months of life, he had marked decline in mobility, increased slowness and substantial swallowing and speaking problems. He was nearly anarthric. Thinking was impaired. He did not have excessive daytime sleepiness, orthostasis, depression or hallucinations. In the last several weeks, oral intake markedly declined on hospice care at home. He died from dehydration. He never had any benefit from reasonable doses of levodopa.
