(PHF1 IHC) ---- Neuro Final Diagnosis: Progressive supranuclear palsy. ---- Neuro Diagnosis Comment: There are no beta-amyloid or neuritic plaques, but some neurofibrillary tangles are seen in an AD-like distribution in the medial temporal lobe. The distribution and density of tangles is consistent with Braak and Braak neurofibrillary tangle stage I (range: I-VI) and amyloid stage 0. These findings exclude a diagnosis of AD by Khachaturian, CERAD, and NIA-Reagan Institute criteria. In NIA-AA criteria, this case also does not meet criteria and is ADNC A0B1C0. In the absence of AD, the tangle pathology has been called ‘primary age-related tauopathy’.
