There is minimal beta-amyloid deposition in this case. (BAP IHC) ---- Neuro Final Diagnosis: Progressive supranuclear palsy; Alzheimer disease neuropathologic change (see comment) ---- Comment: Histological slides of representative neocortical areas show very modest beta-amyloid plaques but no neuritic plaques and only a few neurofibrillary tangles, indicating very early Alzheimer's disease type changes. The distribution of tangles is consistent with stage III (range: 0, I to VI) in the Braak and Braak Alzheimer’s disease staging of tangles and amyloid stage A (range: 0, A-C). These findings indicate the presence of Alzheimer's disease (AD) by Khachaturian criteria, but not by CERAD and the NIA-Reagan Institute criteria.
