Case 9 History
This 75yo female presented at the MDC in 8/1996 for follow-up of asymmetric parkinsonism and worsening of her right-sided tremor. She denied hallucinations, vivid dreams, cramps, dyskinesias, or nausea. She still lived independently but could not take care of her finances. A neurological exam in August 2006 showed the patient was alert and only oriented to person. Her mood and flow of thought were normal. lmmediate recall was correct for 0 items out of 3. The patient recalled 0 of 3 objects at 3 minutes. Affect was appropriate. There were no hallucinations, delusions, suicidal ideation, homicidal ideation, obsessions or compulsions. Extraocular movements were full. Convergence was abnormal. There was no nystagmus. A total of 1 out of 30 was scored on the Mini-Mental State Examination (severe dementia). The patient was unable to listen and then follow paper handling instructions or write a complete sentence. The clinical interpretation was that she had non-progressive, asymmetric parkinsonism stage 3 and Alzheimer type dementia. She died in 9/2009 of a myocardial infarction.
