Case 16 History ---- The patient was a 68-year-old woman with a gait disorder and two years of progressive leg weakness. EMG/NCS showed evidence of a length-dependent sensorimotor axonal polyneuropathy. Operative procedure: Muscle and nerve biopsy. ---- 16A1 Plastic sections show moderate loss of large and small myelinated axons, equal across all fascicles. Many of the remaining large axons show thin myelination for axon caliber, consistent with a history of injury and completed repair. Scattered regenerative clusters suggest more recent injury. There is no evidence of significant intra- and/or inter-fascicular variability, which usually occur in vasculopathy-related neuropathies.
