Case 5 History ---- The patient is a 68-year-old man with a history of relapsed B cell ALL status post dasatinib and sibling matched allo HSCT, who was receiving Ponatinib at the time of biopsy. There was no history of diabetes. He recently developed bilateral painful subacute progressive asymmetric lower extremity (R>L) weakness and numbness. The clinical differential diagnosis was broad but the highest concern was for an autoimmune process vs. Infiltrative process. EMG/NCV shows evidence of very severe sensorimotor peripheral polyneuropathy. Operative procedure: Right sural nerve biopsy
