9A0 Case 9 History
Case 9 History ---- The patient is a 69-year-old man with neuropathic symptoms for 30 years, characterized by dysesthesias in his feet that progressed to his hands which were weak, left more than right. He has had trouble with fine motor movements like buttoning his shirt and opening bags. EMG/NCV is consistent with a primarily demyelinating peripheral polyneuropathy with significant axonal loss in the distal muscles. The uniformity of conduction velocities is suggestive of inherited neuropathy; however, paraproteinemic neuropathy cannot be excluded clinically. Neuromuscular antibody panel showed serum IgM anti-MAG antibodies, which is associated with demyelinating sensory>motor neuropathies, as well as serum IgM anti-TS-HDS antibodies. Operative procedure: Left sural nerve biopsy and left quadriceps muscle biopsy.
