Case 9 History ---- The patient is a 67-year-old man with bilateral foot drop and bilateral neuropathy involving his lower extremities. He is also status post-hip replacement surgery (complicated by metallosis). Operative procedure: Muscle and nerve biopsies. ----
9A1,2 Plastic sections show severe loss of myelinated axons (with loss of large myelinated axons predominating) and both regenerative clusters and myelinated axons with thinned myelin for axon caliber are identified which are evidence of prior axonal degeneration and regeneration. There are scattered macrophages, however there is no evidence of definitive onion-bulb formation, rather aggregates thought to represent POB. (Plastic section)
