26A1 Axonal Loss (small more than large) (Case 26) Plastic 10
The patient is a 58-year-old female with a history of dysesthesias and bilateral distal leg weakness over the past 5 years who developed complaints of worsening weakness of the left foot over the last six months. EMG/NCS showed mild length dependent polyneuropathy and left S1 radiculopathy. Operative procedure: Sural nerve biopsy and quadriceps muscle biopsy. ---- 26A1,2 There is significant axon loss (small > large). A few large axons show thin myelination for axon caliber, suggestive of a cycle of axonal degeneration and regeneration. There are scattered regenerative clusters, which artificially inflate the apparent number of small myelinated axons.
