The patient is a 61-year-old male with lumbar spinal stenosis and distal symmetric dysesthesias which have worsened over the past year. There are no findings to support a demyelinating neuropathy. Operative procedure: Right sural nerve and right gastrocnemius muscle biopsy. ---- 27A1,2 There are numerous unmyelinated axons in this biopsy which are present as singletons (a single unmyelinated axon per Schwann cell unit, arrow 27A1) not as part of Remak bundles. Singletons are a common finding in neuropathies generally in our experience.
