Case 8 History ---- The patient is a 40-year-old woman with a history of gastric bypass and new-onset paresthesias in the left foot greater than right for the past month progressing to the thighs. She then developed a foot drop and numbness in the arms. Serum assay showed a substantial increase in neurofilament light of 1033 (nl < 20pg/mL), and IgG and IgM binding to neurofascin 140 and 155. The clinical differential diagnosis included Guillain-Barre syndrome (GBS). ---- 8A1-3 The whole mount shows apparent vacuoles within the endoneurium as well as numerous crumbs of eosinophilic debris consistent with active axonal degeneration. The vasculature was maintained without evidence of angionecrosis. Additionally, there was no evidence of an inflammatory infiltrate or numerous macrophages. (H&E)
