21J Series of Images: Macrophage-mediated demyelination is characterized by the presence of macrophages within the Schwann cell sheath/basement membrane stripping off otherwise normal myelin from a normal axonal preservation. Such axons were rare, but compelling, in this case. The confident separation of axons from Schwann cell processes in a complex and active milieu in a case with ongoing demyelination and axonal degeneration may be difficult. Also, If there are no myelinated axons, macrophage mediated demyelination cannot be established. Macrophage-derived delicate pseudopods appear involved in the process. ----21J1A-C Demyelination has progressed in this axon to the stage of complete loss of the myelin sheath.
