2A0 Case 2 History
Case 2 History ---- The patient was a 39-year-old man with acute onset of tingling and numbness 11 days ago, along with proximal weakness of his limbs and inability to walk. Symptoms progressively involved his thighs, trunk, and hands, finally extending to his jaw and tongue over the next five days. He had a history of a cold two weeks ago without any history of trauma or vaccination. His past medical history shows similar presentation ten years ago but he declined further evaluation. On physical examination, there was loss of touch and pinprick sensation. The CSF protein was elevated to 140 mg/dl (normal 5-45 mg/dl). There was no abnormal enhancement. These findings were suggestive of a demyelinating process. EMG showed no sensory conduction and normal motor units with reduced recruitment, consistent with severe demyelinating polyneuropathy. Immunologic workup (including ANA, ANCA) is negative. Operative procedure: Muscle and nerve biopsies.
