8A0 Case 8 History
Case 8 History ---- The patient is a 47-year-old male with a history of CIDP diagnosed at age 5. His symptoms - arm and leg weakness, numbness, and associated gait difficulty – were well controlled on steroids since age 27 and azathioprine for the last 10 years; unfortunately, the azathioprine was discontinued in the summer of 2017 due to insurance despite best efforts. From August 2017 until his date of death, he was on IVIG q4 weeks with worsening symptom control – e.g., dropping objects and increased difficulty with transfers due to knee locking. The patient had previously been stable but now was declining steadily. He was wheelchair-bound and lived alone. He was noted to have stable mild respiratory compromise for the last few years. He was last seen in clinic on 6/27, when he was at his baseline, denying fever, flu, chills, cough, or recent illness, rash, wounds, open sores, blisters, or surgical incisions. He had not had any recent hospitalizations or surgeries. He was found deceased by his mother on the evening of 6/29.
