Case 1 History ---- The patient is a 50-year-old woman with a long history of neuropathy and reports recent worsening of symptoms with marked upper and lower extremity weakness, back pain and paresthesias throughout bilateral upper and lower extremities. In addition, she has a history of lupus, epilepsy, fibromyalgia, rheumatoid arthritis, and celiac sprue. NCV shows evidence for a mixed axonal and demyelinating peripheral neuropathy. MRI shows moderate diffuse atrophy of the thoracic cord and diffuse cerebral atrophy, more advanced than expected for her age. Operative procedure: Right quadriceps muscle biopsy and right sural nerve biopsy.
