At autopsy the weight of the unfixed brain was 1260g. Granulomatous inflammation was identified in the liver, hilar lymph nodes, abdominal lymph nodes, and the CNS. ---- 4A1 The gross brain was remarkable for opacity and focal nodularity of the leptomeninges, particularly those of the brainstem and spinal cord. Multiple space occupying and coalescent granulomas were associated with the leptomeninges, Virchow-Robin spaces, the hippocampus, brainstem, cerebellum, and spinal cord and provides an anatomic explanation for the increased intracranial pressures and seizures that the patient experienced during life. AFB and GMS stains for acid-fast and fungal organisms were negative in both the hilar node section and in two central nervous system sections, providing further support for the diagnosis of sarcoidosis.
