The surrounding perineurium appears hyperplastic with increased cytoplasm including increased numbers of mitochondria in perineurial processes(?). (electron micrographs) ---- Comment: The pattern of axonal loss in the nerve biopsy material is consistent with an ischemic pathogenesis. No direct evidence of ongoing vasculitis or amyloid deposition is identified, but widespread vasculopathic changes are present, and subperineurial edema and
focal perineuritis are noted. The etiology of the vasculopathic changes is unclear.
