Case 13 History ---- The patient is a 30-year-old pregnant woman with diabetes, weakness, cramping and burning paresthesias in bilateral lower extremities. Electrodiagnostic testing showed a patchy, multifocal, axonal polyneuropathy. CSF studies showed slightly increased protein and normal cell count. There was clinical concern for CIDP or mononeuritis multiplex. Operative procedure: Left quadriceps muscle biopsy; left sural nerve biopsy. ---- Not shown: Toluidine blue stained plastic sections show active Wallerian degeneration involving most myelinated axons with a few remaining smaller myelinated axons. ---- 13A1-3 The perineurium shows numerous focal calcifications of unknown pathogenesis.
