Case 14 History ---- The patient was a 60-year-old female with past medical history significant for hypertension, type 2 diabetes, dyslipidemia, who recently had a cholecystectomy. One week post cholecystectomy she had progressive worsening of her mental status and poor functional status with increasing anxiety, nervousness, and significant major depression causing suicidal ideation. She had a CT scan and MRI done as an outpatient which showed mild atrophy and, due to declining functional status, she was admitted to the hospital. There was some focal numbness on examination and some weakness of lower extremities, causing her inability to walk. The patient had multiple radiological studies and a neurologic consultation. An LP showed positive 14-3-3 protein. All other workup was negative. The family decided for hospice care and the patient passed away with a total duration of 3 months. ---- At autopsy the weight of the unfixed brain was 1260g. The external and cut sections of the cerebral hemispheres, cerebellum and brainstem were grossly unremarkable. ---- 14A1 This DWI MRI image shows hyperintensities involving the caudate, putamen and pulvinar nuclei bilaterally.
