Case 1 History ---- The patient was an 80 year old woman with a history of dementia and hypertension who died of respiratory failure following surgery for volvulus of a paraesophageal hernia. ---- 1A1,2 There is a modest degree of expansion and tortuosity of the vertebrobasilar arteries (arrowhead). Although the patient had an unexplained medial gaze palsy, microscopic examination of the brainstem failed to demonstrate focal structural pathology.
