Within the wall of the aneurysm there is a fragment of elastic lamina shown at high magnification (VVG). ---- Denouement: This likely represents the expansion of a saccular aneurysm in the anterior cerebral artery/pericallosal circulation that eventually ballooned secondary to his known chronic hypertensive disease and underwent thrombosis as a sequela. The aneurysm corresponds to pericallosal location which is consistent with his prior angiograms. Nevertheless, this is a relatively unusual location. The aneurysm was not ruptured, although there were chronic changes associated with thrombus formation and its organization, mural calcifications and presence of blood breakdown products both in its wall and immediately surrounding brain parenchyma.
