Case 1 History ---- The patient was a 50-year-old man with a recent history of osteomyelitis of his right fourth toe accompanied by nausea, vomiting, weakness and fever. Blood cultures were positive for methicillin-resistant Staphylococcus aureus. His past medical history was significant for an aortic valve replacement in 1990, diabetes mellitus type II, hypertension and stroke. He was transferred to BJH for treatment of an aortic root abscess that was visualized by echocardiogram. He received intravenous antibiotic therapy and prophylactic anti-coagulation. On 10/8/03, a head CT scan was performed on the patient as part of pre-operative study and showed a left parietal acute intraparenchymal hemorrhage adjacent to an old CVA. He did not present new neurological deficits but these findings made surgical intervention impossible. The patient was found asystolic and apneic one week later.
