Case 2 History ---- The patient is a 60-year-old man with a complicated history of thyroid cancer who was admitted to the medicine service for treatment of endocarditis and diskitis. His blood cultures grew oxacillin-sensitive staphylococcus aureus. He experienced some mental status changes, as well as some right facial weakness. A CT scan and subsequent MRI demonstrated a large left frontal cystic mass consistent with abscess with significant surrounding edema. In addition, multiple smaller lesions consistent with emboli were noted throughout the brain. The patient underwent Stealth CT with contrast for intraoperative image guidance. Intraoperative inspection was consistent with a probable mycotic aneurysm along the anterior aspect of the left frontal abscess.
