3E Aneurysm, mycotic, Case 3) SMA WM
A smooth muscle actin stain shows numerous myofibroblasts but no definite normal vessel wall structure. (SMA IHC) ---- Special stains for microorganisms AFB (Acid fast bacilli), GMS (Gomori Methenamine Silver) and Gram stain are negative. ---- Comment: The morphologic, histochemical and immunohistochemical features are consistent with an aneurysm with an organizing thrombus which, at the time of specimen collection at least, does not contain definitively identified microorganisms as expected in an infective “mycotic” aneurysm. It doesn’t meet the gold standard of aneurysm with bacterial/fungal organisms.
