Dissection of the circle of Willis shows a 1.8-cm broad-based aneurysm at the junction of the right internal carotid artery and right posterior communicating artery that was adherent to the brain parenchyma. We did not see direct evidence of aneurysm rupture; however, such foci can be difficult to identify.
