Case 20 History ---- The patient was a 46 year old woman with no significant past medical history who underwent abdominal hysterectomy following a diagnosis of cervical carcinoma. One day later she developed a headache (BP180/90) and lethargy over several hours. She developed Cheyne-Stokes respirations and had a cardiac arrest. EMI scan revealed blood in 3rd and 4th ventricles but none in the parenchyma. A ruptured 1.5 cm vertebral aneurysm of the distal right vertebral artery was demonstrated as well as extensive spasm of the posterior and anterior circulation. She became more obtunded and died. ---- The vertebral artery aneurysm (arrow) is located immediately adjacent to and presumably compressing the right side of the medulla
