Case 22 History ---- The patient was a 46-year-old hypertensive woman. During the workup of a subarachnoid hemorrhage, a ruptured left posterior inferior cerebellar artery aneurysm which was discovered to be spontaneously obliterated. The patient received a craniotomy and clipping of the aneurysm. The other aneurysms that were discovered incidentally at that time included aneurysms of the right middle cerebral, anterior communicating and left posterior communicating arteries. The three incidental aneurysms were operated upon on 6-12 at St. Anthony's Medical Center. The right MCA aneurysm required two clips, the anterior communicating artery required one clip and the left posterior communicating artery required one clip as well. The patient also had a subsequent VP shunt placed in the posterior right lateral ventricle.. She returned to an OSH with Pseudomonas bronchitis, SIADH and anorexia. Various CT scans during the course of her hospitalization demonstrated ischemic infarction of the right frontal, parietal and temporal lobes. She also had an extensive infarction on the left cerebellum. She continued in a vegetative state and was admitted to BJH in February of 2001 for chronic pulmonary infections. She was admitted to the medical ICU, but subsequently decided to withdraw care and receive full hospice care. She expired on 3-9.
