Washington University Experience | VASCULAR | Aneurysm - Saccular | 4A2 Aneurysm, Basilar-Vertebral, coiled (Case 4) 10
4A2-4 The lumen of the aneurysm contains a conglomerate of delicate wires and admixed blood clot. After careful dissection using a stereomicroscope, the examination showed that the external surfaces of the aneurysm wall over the dome to be intact. An incision was made in the dome of the aneurysm and the congerie of delicate wires and blood clot are taken out from this intact surface. The aneurysm is then bisected to show the site of origin of the aneurysm as well as its apparent point of rupture, which is represented by transmural hemorrhage in an area measuring 0.3 cm in diameter. This apparent rupture point is located towards the neck of the aneurysm on the superior aspect, facing the brain surface. The aneurysm wall is no longer intact in the area grossly described as the bleeding site and has been replaced by transmural blood clot and fibrin which extends completely through the wall at this point. There are no wires identified within the rupture site itself.
