16A0 Case 16 History
Case 16 History ---- The patient was a 69 year old, right-handed male with a past medical history significant for ulcerative colitis for which the patient was treated with prednisone and 6-MP. He developed cryptococcal meningitis, and was admitted to BJC for one month. Complications of his recent hospitalization included deep vein thrombosis for which the patient was treated with lovelock. He also had decreased mental status, left hemiparesis, and aphasia attributed to cryptococcal meningitis. MRI during that hospitalization showed no evidence for infarction. In addition to the cryptococcal meningitis, the patient had ORSA, septic arthritis, and fungemia. He was readmitted on three days later after he found unresponsive at Barnes Extended Care Facility. His examination on admission showed a temperature of 39oC. He was intubated on admission. There was no evidence of brain stem activity, except spontaneous respirations above the ventilator. Head CT scan showed multiple new parenchymal hematomas, right thalamic hemorrhage with rupture into third and fourth ventricles with obstructive hydrocephalus. His family withdrew support and he was extubated and later died.
