6A0 Case 6 History
Case 6 History ---- The decedent was an 83-year-old woman with a past medical history significant for hypertension, type 2 diabetes, and Alzheimer's disease. Per her niece, the patient was at her usual state of health up to March to April of 2015, when the family noticed a decline in her status a few months later and she was diagnosed with Alzheimer's disease. In April 2016, the patient was reported to have fallen twice at home and was found to have an L1-L5 fracture and was discharged to a skilled nursing facility three days prior to the day of expiration. The patient fell out of her wheelchair while nursing home staff were at her side who noted she was in cardiac arrest and started performing CPR and bag mask ventilation within 1 minute of arrest. There was spontaneous return of circulation in the ambulance bay, however, pulses were lost on route to the hospital. EKG was concerning for an ST elevated MI and anterior infarct. In the Emergency Room there was cardiac standstill and no pericardial effusion identified on ultrasound.
