13A0 Case 13 History
Case 13 History ---- The patient was a 58-year-old woman with a two-year history of psychosis, bizarre behavior, hallucinations, personality changes and deterioration in her mental status; MMSE score was 26. Neurological exam identified a right-sided positive Babinski sign and a left pronator drift. Her family history was significant for a mother with Alzheimer disease, developing at around age 80. Magnetic resonance imaging (MRI) of the patient’s brain showed extensive cerebral and cerebellar microhemorrhages and associated vascular disease, as well as scattered areas of punctate parenchymal and leptomeningeal enhancement. Clinical diagnosis: Cerebral amyloid angiopathy vs. central nervous system vasculitis vs. infection. Operative procedure: Right frontal burr hole and open brain biopsy.
