11A0 Case 11 History
Case 11 History ---- The patient was a twenty-eight hour old female infant (Twin B), born at 24 weeks EGA to a 24 year old G1P2 mother. The amniotic membrane broke spontaneously, and the mother spontaneously vaginally delivered a second limp and blue female infant. This patient was intubated and positive-pressure ventilation was given. A chest x-ray performed, which showed bilateral lung white-out. Antibiotics were given. She received a dose of surfactant and remained on high ventilatory support. She continued to have worsening blood gases and persistent metabolic acidosis. A chest x-ray showed bilateral pulmonary interstitial edema. The patient became increasingly hypoxemic and her condition continued to worsen despite multiple rounds of cardiopulmonary resuscitation and died.
