Case 16 History ---- The patient was a 28 hour old male infant EGA 26 weeks born to a 22 year old G3P1 woman. Prenatal complications included twin pregnancy, limited prenatal care which began only one week prior to delivery with a maternal UTI and preterm labor on 9/28. The patient was born as the second twin at 1343 with an Apgar score of 4 at 1 minute and 8 at 5 minutes. He was intubated with positive pressure ventilation but his course was complicated by progressive hypoxia and acidosis despite maximum ventilatory support and hypoglycemia and hypotension despite fluid and inotropic medications. Chest x-ray showed features consistent with pulmonary interstitial emphysema. In view of the poor prognosis, ventilatory and IV support were withdrawn and the patient expired on 9/29.
