Case 19 History ---- The patient was a two day male infant born at 24 weeks gestation with a gestational weight of 618 gm, to a 33 year old mother. The pregnancy was unremarkable. This infant had developed severe respiratory distress within the first several hours of life. He was treated with surfactant. A chest X-ray showed severe hyaline membrane disease. Subsequently the infant developed pulmonary interstitial emphysema and pneumothorax, requiring chest tube drainage. A second episode of pneumothorax occurred the next day. After successful drainage of the second pneumothorax the infant developed profuse bleeding from the endotracheal tube. The clinical impression and X-ray appearance was consistent with pulmonary hemorrhage, showing a positive blood bronchogram. The terminal phase was dominated by bradycardia. The infant died on 12/17 after 2 days of life. ---- At autopsy periventricular infarction and hemorrhage involved the lateral, the third and fourth ventricle, with extension into the subarachnoid space of the posterior fossa.
