30M Denouement ---- The brain in this patient shows the pathology of the long term residua of extensive prematurity resulting in a large germinal matrix with extension into the parenchyma and ventricles. Intraventricular hemorrhage induced extensive granular ependymitis throughout the entire ventricular system, perhaps with other sites of hemorrhage. The cortex does not show multiple infarcts nor does the white matter have typical focal necrosis and calcification of periventricular leukomalacia. One unusual feature in this case is the presence of marked cerebellar atrophy, in particular the right cerebellar hemisphere which appears to reflect the marked damage to the left cerebral cortex and the induction of pontine atrophy and “crossed cerebellar atrophy”. Since the opposite cerebral right hemisphere also shows significant damage, the loss of significant parenchyma of the left cerebellar hemisphere may reflect, at least partially, the contribution of a similar process.
