4A0 Case 4 History
Case 4 History ---- The decedent was a male infant born at 25 weeks EGA who developed respiratory distress with difficult to control ventilation and oxygenation. Around 11 days of life, his respiratory status worsened and he developed metabolic acidosis. A tracheal aspirate was positive for Enterobacter and antibiotics were broadened, but he became coagulopathic, and developed bilateral grade IV intraventricular and intraparenchymal hemorrhages and, after a few hours, developed seizures not responsive to phenobarbital or fos-phenytoin. Repeat head ultrasound showed worsening of the bleeds, and he continued to intermittently seize. The family decided to redirect care. ---- Autopsy showed intraventricular hemorrhage with bilateral parenchymal extension (Grade IV), subarachnoid hemorrhage at the base of brain and subpial hemorrhages in the cerebellum.
