Case 5 History ---- This child was a 2 and a half month old female who was transferred to SLCH for workup and management of post hemorrhagic hydrocephalus. She was born at 27 weeks via cesarean section with the pregnancy complicated by tobacco use and + Group B strep culture and placental abruption. Initial APGARS were 7 and 8. A head ultrasound on DOL #1 revealed intracranial hemorrhage with subsequent hydrocephalus which was initially treated with serial taps but eventually underwent VPS shunt placement. Her neurologic course was also complicated by GTC seizure activity. Upon transfer a brain MRI revealed mod/severe hydrocephalus mainly of 3rd and lateral ventricles, some intraventricular and subependymal blood, with porencephalic changes in the right frontal lobe. She developed progressive respiratory failure and ascites, for which her shunt was externalized 12/16 to facilitate peritoneal dialysis. This was not successful however and care was withdrawn 12/17.
