Case 2 History ---- The patient was a 39 year old woman with mixed connective tissue disease diagnosed 1 year before her death with progressively worsening pulmonary hypertension resulting in right heart failure not responsive to medical therapy, who underwent planned atrial septostomy. During the procedure there was right ventricular perforation, with tamponade, which was repaired. She was hemodynamically unstable for the rest of her course and died approximately 10 days later..---- The cerebellum appeared focally discolored and was soft to palpation.
