5A0 Case 5 History
Case 5 History ----- The patient was a 43-year-old woman with a past medical history of HIV/AIDS, hepatitis C, poly-substance abuse, depression and methicillin resistant Staphylococcus aureus pneumonia. She presented to the ED following 3 days of decreased oral intake after she was found unresponsive at the nursing home with O2 saturations in 80’s. She was treated with supplemental oxygen, started on antibiotics for pneumonia and admitted to the medicine floor. On initial neurological examination, she was described as oriented times 2, with reactive pupils and symmetric reflexes. Two days later was found to be in asystole and without spontaneous breathing by the nursing staff. Nine minutes after starting CPR, she regained heart rhythm. The neurological exam 3 days later reported nonreactive pupils, no dolls, corneal or gag reflexes and no motor response. Her family decided to withdraw care 2 days after this and she died.
