Case 8 History ---- The patient was 5 month old male with CHARGE association, including choanal atresia, hiatal hernia, multiple heart anomalies, DiGeorge syndrome, supraventricular tachycardia, seizure disorder, hydrocephalus, and acute respiratory distress syndrome who expired after elective termination of ECMO support which showed no clinical benefit after 16 days.
The baby was limp, blue, and apneic at delivery, requiring intubation at 1 minute of life. Apgar scores were 5 at one minute and 7 at five minutes. A total IgG as a screen for TORCH infections was normal. Heart dysfunction was significant for a patent foramen ovale with bowing of the atrial septum and left to right shunting, right ventricular hypertrophy and dilatation, mild tricuspid and mitral regurgitation, bicuspid aortic valve, mild valvular and supravalvular aortic stenosis, and a patent ductus arteriosus with bilateral shunting. The patient’s course was stormy with difficulty with cardiac and pulmonary dysfunction that lead to his death.
