Case 15 History ---- The patient is a 69-year-old female with a past history of hypertension, type II diabetes, hyperlipidemia, obstructive sleep apnea, coronary artery disease, and chronic ischemic left middle cerebral artery stroke who presented to an emergency department in cardiac arrest in July 2026. She had previously called emergency medical services (EMS) saying she was not feeling well and reported chest pain. Upon EMS arrival, the patient was apneic and pulseless. Her EKG showed pulseless electrical activity. Resuscitative measures were initiated by EMS and continued upon arrival to Barnes Jewish Hospital Emergency Department. A bedside ultrasound showed no cardiac activity; her end tidal carbon dioxide was 9, lactate was 12.9, and pH was less than 6.8. Resuscitative measures were performed for approximately 45 minutes in total with low probability of meaningful recovery and the patient passed away.
