The right hemispheric lesion involves the right frontoparietal lobes and the basal ganglia. The probe indicates the position of the MCA embolus which unfortunately is hard to see. The appearance of the right ACA/MCA “fracture” in this case is artefactual but reflects the failure of the infarcted MCA territory to firm up in fixative and the fracture is related to the cutting process.
