Case 12 History ---- The patient was a 74 year old man who was found unconscious by his family on 11/29. In the ER, a head CT revealed a left middle cerebral artery cerebrovascular accident. The patient was also noted to have a blood glucose of 500 and was treated for diabetic ketoacidosis. He was aphasic and unable to provide any further history. On 12/2 the patient deteriorated. His pupils were fixed and dilated with minimal corneal reflexes. The patient was intubated. A head CT showed subfalcine herniation with a large midline shift and extension of an infarct in the anterior circulation. Two brain death exams were performed and the patient passed away on 12/5.
