2A0 Case 2 History
Case 2 History ---- This 91-year-old man was seen by a neurologist with an 11 year long history of gradual cognitive decline. On 7/30 he was found unresponsive on the floor at his assisted living facility and taken to an OSH ER. Initially, he had left hemiparesis with aphasia and was evaluated for an acute stroke. However, CT and MRI showed no areas of cerebral infarct but revealed a compression fracture of the C5 vertebral body with a large posterior disc herniation at C4-C5 and marked narrowing of the spinal canal. Shortly thereafter, he stopped moving all extremities and lost consciousness. The family decided to discontinue life support and the patient died on 7/31 one day after onset of his illness. ---- At autopsy his brain weighed 1030g. A diagnosis of Alzheimer and argyrophilic grain diseases were made. The cord section showed pronounced compression with necrosis/ischemia.
