Case 18 History ---- The patient was a 71 year old man with a past medical history of: hypertension, hyperlipidemia, congestive heart failure, ischemic cardiomyopathy, coronary artery disease (status post multiple bypass surgeries), renal artery stenosis, deep venous thrombosis, and a cerebrovascular accident in 1996 with residual left sided hemiparesis. The patient's most recent MRI of the brain (dated February 2008) shows an old lacunar infarct involving the right basal ganglia and internal capsule with associated Wallerian degeneration of the right cerebral peduncle. Terminally, he presented to BJH from an extended care facility with a one week history of progressive shortness of breath. Chest X-ray showed findings consistent with pulmonary edema and a small left pleural effusion which did not improve with therapy and he passed away.
