16A1 Infarct, subacute (Case 16) b copy
Case 16 ---- The patient was a 94 year old woman transferred from a nursing home on 10/22 for management of an arm wound. She had been recently admitted to an OSH from 9/13-9/30 for aphasia and right hemiparesis due to a large left middle cerebral artery infarct involving frontal and parietal lobes and periventricular white matter changes on MRI. Her past medical history was significant for an MI in 1999, hypertension, and dementia. During hospitalization, she aspirated, developed a fever and became septic with pulmonary infiltration, yeast in urine, C. difficile in stool and renal failure. She became apneic in the CT scanner and was pronounced dead on 10/25. ---- 16A1,2 The gross images of the exterior surface of the brain (arrow, 16A1) show an unusual friability and degeneration which is unusual.
