The monocyte/macrophage/microglial lineage marker CD163 stains macrophages in the infarct margin and microglial hyperplasia in the cortex. (CD163 IHC) ---- Comment: The collective morphological and immunohistochemical features are consistent with a subacute infarct; the etiology of which could not be ascertained from the current biopsy. There is presence of perivascular inflammation, which may be present in association with infarcts. Absence of angionecrosis or vessel wall destruction by inflammatory cells outside the area of immediate infarction argues against a vasculitic process but vasculitis may be focal.
