Case 4 History ---- This 56 year old man had a lengthy ischemic cardiac history. He was waiting for a heart transplant but, unfortunately, developed a stormy terminal course that involved use of a ventricular assist device. Hepatic dysfunction resulted in hyperbilirubinemia which marked areas of infarction green. The precise time of infarction was not clear by history but likely occurred 14-16 days prior to death. ---- A minute infarct with green bilirubin staining occupies the depths of a small sulcus.
