Case 15 History ---- The decedent was an 86 year old man with longstanding hypertension, cigarette smoking and vascular disease in the lower extremities. He had a stroke (generalized weakness, dizziness, blurred vision) two years previously. At the end of February 2002 he had fallen at a nursing home and developed rhabdomyolysis and dehydration post-fall. He was evaluated at BJH and rated CDR 0, no dementia. He did not develop dementia during his entire course and terminally was rated CDR 0. ---- At autopsy the weight of the unfixed brain was 1150g. His final diagnosis was diffuse Lewy body disease with idiopathic Parkinson’s disease, even though he had neither history of dementia nor parkinsonism. There was severe cerebral atherosclerosis with marked basilar artery stenosis and a left cerebellar infarct. Arteriosclerosis was patchy throughout the white matter and basal ganglia.
