A low magnification image of the left ION shows a patchy transneuronal effect in which the dorsal part of the ION (arrow) retains its normal appearance and the ventral portion (arrowhead) has undergone a hypertrophic response. The patchy hypertrophic change likely reflects the patchy effect of the malformation on descending fibers of the CTT. (H&E)
